What is Adult Mental Health Case Management?
Individual case management services are designed to assist adults with a serious and persistent mental illness gain access to needed medical, social, educational, vocational and housing services within the community.Case managers meet with clients at a variety of community locations; including residences that are convenient and easily accessible for those involved in the program.

Who is Eligible
Funding Prior to services, payment or insurance will be authorized. Client must be Medicaid eligible, a county resident, and/or self pay for services.
Individual case management services are designed to help children with severe emotional disturbance and their families obtain needed mental health, medical, social, educational, and vocational services within the community.
Case managers meet with clients and their family at a variety of community locations, including residences, that are convenient and easily accessible for those involved in the program.

1 in 5 children, ages 13‐17, have or will have a serious mental illness. Knowing this,
it is important for parents, guardians, family members, teachers and/or others
to watch for signs of mental illness in children. These changes can include withdrawal, sadness,
anxiety symptoms, mood swings, excessive fear, loss of interest in activities they use to enjoy and/or even self harm to him or herself. If a change in their behavior is recognized, it should be addressed
immediately. Talk with the child, parent/guardian, doctor, teacher and/or others who are involved in the child’s
care to discuss these behaviors. For assistance with obtaining help with these behaviors, call or fill out a referral on our website and a EFCS’s staff will help you to set up an appointment with a
mental health professional to discuss the needs of
the child.
There are many different types of mental illnesses that an individual can experience. Statistics report that one in four adults experience a mental illness in a given year. Sign of mental illness may include, sadness, anxiety, stress, mood swings, loss of interest, anger or withdrawal. If you or a loved one have been experiencing any signs and others, or are in need of assistance with everyday living skills, EFCS can link you or your loved one to a variety of Community services that can help you diagnose, treat and understand your illness.
